Media Release Forms
Before attending certain GLOBE events or functions, participants must sign our Media Release Forms. To complete the media release process:
- Select the preferred media release form below.
- Media Release Form for ADULTS (fillable PDF)
- Media Release Form for ADULTS (online submission)
- Media Release Form for MINORS (fillable PDF)
- Media Release Form for MINORS (online submission)
- Fillable PDF: Download the form.
- Print the form and complete it in pen. Scan the form and email it to communications@nasaglobe.org
- Fill in the PDF form on your computer. Save the file and send it to communications@nasaglobe.org
- Online submission: Fill out the online form and click Submit.
Important: Make sure to sign and complete both pages of the Media Release Form document. Page one contains a NASA-specific form while page two contains a GLOBE-specific one. You must sign both forms to complete the media release process.
