Media Release Forms

Before attending certain GLOBE events or functions, participants must sign our Media Release Forms. To complete the media release process:

1. Download the Media Release Forms onto your device
   • Media Release Form for Adults
   • Media Release Form for Minors
2. Complete the forms on your device or print out the forms and fill them out manually
   • If you manually completed the forms, scan the finished copies to your device and save it as a PDF
3. Send the signed and completed release forms to globecommunications@ucar.edu

Important: Make sure to sign and complete both pages of the Media Release Form document. Page one contains a NASA-specific form while page two contains a GLOBE-specific one. You must sign both forms to complete the media release process.
